COVID-19 Liability Release and Screening
Waiver of Liability Relating to Coronavirus/COVID-19
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is reported to be extremely contagious. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and even death.
Disciples Crossing Camp cannot prevent you [or your family, or group] from becoming exposed to, contracting, or spreading COVID-19 while utilizing Disciples Crossing’s services or premises. It is not possible to prevent against the presence of the disease. Therefore, if you [and your group] choose to utilize Disciples Crossing’s services and/or enter onto Disciples Crossing’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19.
ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose to accept the risk on behalf of my group of contracting COVID-19 for myself and/or my family in order to utilize Disciples Crossing’s services and enter Disciples Crossing’s premises. These services are of such value to me [and/or to my family and group] that I accept the risk of being exposed to, contracting, and/or spreading COVID-19 in order to utilize Disciples Crossing’s services and premises in person.
WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive mine and my group’s right to bring suit against Disciples Crossing and its owners, officers, directors, managers, officials, trustees, agents, employees, or other representatives in connection with exposure, infection, and/or spread of COVID-19 related to utilizing Disciples Crossing’s services and premises. I understand that this waiver means I give up my right to bring any claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or unforeseen.
COVID-19 Pre-Participation Screening
It is the group leader’s responsibility to inform all individual participants of Disciples Crossing’s policy and determine if they will perform screenings for all participants.
Face coverings and social distancing guidelines are welcome, especially while indoors, and shall be determined and enforced by the group leader.
The following are Disciples Crossing’s recommended screening questions.
Have you, in the past week, experienced any of the following new or worsening signs or symptoms:
Fever measured at greater than or equal to 100.0 degrees Fahrenheit OR one of the following symptoms: cough, shortness of breath, difficulty breathing?
Any of the above listed symptoms AND any of the following: chills, repeated shaking with chills, muscle pain, headache, sore throat, loss of taste or smell, diarrhea?
Have you been in contact with anyone who has been diagnosed with COVID-19 and/or someone who is under investigation for having COVID-19?
